Sometimes your health insurance company may require you to submit a prior authorization (PA) form to process your medication(s) through insurance. This is to get confirmation from your health insurance that your treatment plan is medically necessary. 


To initiate a prior authorization request, you can submit the request with your provider by messaging them in the patient portal, or by emailing our customer support team for assistance. 


The process will begin by collecting your insurance information, if it's not already on file. Please make sure to have this information uploaded to your portal when initiating a prior authorization request.

3) RxPCN#, 

4) RxGroup #

5) Photo of the front and back of your insurance card



Please note that when your prior authorization request is submitted, this does not guarantee that your health insurance company will cover the cost of your medication(s). If your PA request is denied, you may need to pay out-of-pocket or work with your provider to find alternative medication(s) that are covered by your insurance. 


If you have insurance, we encourage you to call your insurance company before your appointment to confirm the list of covered medications. Please note that the prior authorization process is can take up to 10 business days and is heavily dependent on how timely your health insurance responds.
