It is best to discuss your long-term treatment plan with your healthcare provider who is familiar with your medical history, diagnosis, and symptoms. Your healthcare provider will create an individualized treatment plan that is tailored to your specific needs and may include various components, such as medication, therapy, lifestyle changes, dietary changes, and support groups. If you have any additional questions regarding your treatment plan, it's best to speak with your healthcare provider directly.

An initial visit with your provider will usually include a prescription, if appropriate. Starting from the second visit, your provider will refill your prescription and constantly monitor your dosage and changes to your condition. The refill request* is $25; if the provider needs to have a follow-up visit with you the cost is $59 per visit. Keep in mind your medication cost is separate to the appointment service fee. Remember, you don't need to sign up for a subscription and you can cancel services.

The long-term treatment plan includes:

1) Medication Management

In general, a provider prescribes and refills your medication. You can pick it up from your preferred local pharmacy.

2) On-Going Care

You'll receive access to your secure client portal where you can easily message your provider, get your questions answered, and receive related documents. Your provider will follow up with you periodically to get an update on your health and optimize your treatment if necessary.

3) Customer Support & Care Coordination Access

You may converse with the Hello Klarity customer support admin team or care coordination team for concerns like appointment scheduling, insurance questions, billing issues etc. We will do our best to help!

4) Cancel Anytime or Switch Provider

You may cancel services at any time. If you would like to switch providers, our care coordination or customer support team will help you during the transition.**

*Refill requests ($25) may not be available in certain states.
