When transferring to a new provider, a completely new relationship needs to be established. Please keep in mind that each provider may have different practice requirements. A medical release consent form will be sent to your email to give the new provider access to view your medical records. (if applicable)


If they approve to take you on as a patient, they will review your medical history and make their determination on an appropriate treatment plan during your visit.


The appointment itself is discounted $90 from a normal initial visit cost of $149 to a follow-up fee of $59. While customer support will do their best to accommodate your request for a new provider, starting the transfer process does not guarantee continued treatment. We encourage you to communicate your concerns with your current provider before requesting a change to ensure that all available options have been explored.


It is entirely at the discretion of the medical practitioner to decide whether to accept a patient, to continue or modify treatment, or to change the treatment plan entirely. As a patient of the new provider, you will be required to comply with any practice requirements which may include in-person visit(s), EKG, and/or blood pressure readings. Please communicate with the provider in the client portal with any questions regarding their practice requirements. Klarity customer support cannot guarantee transfer or treatment outcomes.
